- W4387396543 abstract "Male infertility affects almost half of the patients seeking fertility treatment, and thus semen analysis is an essential part of diagnosing and treating patients. Production of semen for semen analysis is typically done on-site to reduce sample degradation that occurs in the time elapsed between collection and analysis/usage. However, collecting semen samples on-site at a fertility center's collection room can be a challenge for patients, as it can cause stress, discomfort, and scheduling conflicts while existing containers for at-home production can be cost-prohibitive. We designed and investigated the feasibility of an at-home semen collection kit for clinic drop-off that is cost-effective, reliable, and convenient. We developed a low-cost (<3 USD) at-home sperm collection kit, the Sperm Transport Container (STC), comprised of a standard sterile specimen cup placed in a customized insulated Styrofoam box that holds the container at a 45-degree angle to reduce surface area and evaporation and slow the rate of cooling. A refrigerant-free buffer is added to the cup after sample production to maintain the viability and motility of sperm during transport. Sperm survival parameters in pooled, deidentified semen samples set to be discarded were measured after 3 hours in the STC (n=5). Sperm survival parameters were also measured after 24 hours and the rate of heat loss was compared using the STC, a sterile collection cup alone (cup), and the commercially available ProteX cup (∼70 USD). Following a 3-hour incubation in the STC, there was no change in sperm morphology or count and only an average 5% decrease in forward progression and an 8% decrease in motility. After 24 hours, the forward progression decreased by an average of 53% for the STC and 64% for the ProteX cup (p=0.54). The total motility after 24 hours decreased by an average of 13% for the STC and 14% for the ProteX cup (p=0.98). Over the initial 10 minutes, the rate of heat loss from 37.3°C was 0.43°C/minute for the Transport Container, 0.47°C/minute for ProteX, and 1.07°C/minute for the sterile cup (p=0.04 for STC vs cup, p=0.05 for STC vs ProteX). Sample temperatures equilibrated with room temperature at 90, 60, and 20 minutes using the STC, ProteX, and standard collection cup, respectively. The STC had a slower rate of heat loss and comparable retention of semen analysis parameters compared with commercial products costing ∼20x more. The STC can provide a cost-effective and convenient method for at-home semen production that can expand access, and will allow clinics to offer a comfortable and convenient option for patients requiring a semen analysis while maintaining the integrity and quality of sperm samples." @default.
