- W4387396670 abstract "PCOS is associated with an increased risk of pregnancy complications including miscarriage, gestational diabetes mellitus (GDM), hypertensive disorders of pregnancy (HDP), and preterm birth (PTB). This study aimed to assess whether patients with PCOS received counseling specifically related to pregnancy complications. We performed an online survey of patients with PCOS and a prior pregnancy or those attempting pregnancy. The validated survey collected demographic characteristics, pregnancy complications, and counseling experiences of the participants. Chi-squared tests were performed to compare the proportion of patients receiving specific pregnancy counseling by race. Demographic data for 302 participants is shown in the Table. 72.8% respondents had a prior pregnancy and 66.7% of these reported complications during pregnancy. Only 52.8% reported receiving counseling on pregnancy complications related to PCOS prior to attempting pregnancy and 41.5% were satisfied with the information received. The most frequently discussed complications were miscarriage (65.7%) and GDM (68%), while HDP (33.1-39.5%), and preterm birth (23.8%) were discussed less frequently. Although there were no racial disparities in overall counseling, more Black patients were counseled about Cesarean section (p=0.03), preterm birth (p=0.01), and preeclampsia (p=0.01). In a sub-analysis of patients who were prescribed ovulation induction medications and hence had a pre-pregnancy provider visit (n=243), only 59.3% received counseling about pregnancy complications. Overall 43.4% received counseling about pre-pregnancy weight management; however, only 38.1% were satisfied with the information received. Less than 7% reported receiving PCOS specific information about complications during the postpartum period and/or lactation. Although 83.1% reported a history of anxiety or depression, 60% were not provided with any pregnancy specific mental health information and only 56.5% were satisfied with the information received. Despite the recognized pregnancy and postpartum complications associated with PCOS, our study reveals a significant lack of patient counseling about the ante and postpartum period combined with dissatisfaction about the information received." @default.
