- W4387410332 abstract "To determine the association between ethnicity and the decision to pursue and utilize autologous cryopreserved oocytes. Retrospective Cohort All autologous oocyte cryopreservation cycles for elective fertility preservation at one institution from 2010-2020 were identified. These datasets were further divided into patients who pursued cryopreservation after an initial consult appointment, and patients who thawed and utilized cryopreserved oocytes. Among subjects who thawed cryopreserved oocytes, the sperm source (partner vs. donor) was also identified. Based on self-reporting at initial consultation, patients were categorized into five ethnicity cohorts: Asian, Black or African American, Hispanic or Latino, White, Unknown/Other/American Native. These categories were based on the 2015 NIH Diversity reporting recommendations. Individuals seeking cryopreservation for oncofertility or medically-indicated reasons were excluded. A total of 5915 women pursued initial fertility preservation consultation at our institution over a 10-year timeframe. Of those, 3504 moved forward with oocyte cryopreservation cycle(s) and 2411 did not. Ethnicity was significantly associated with oocyte cryopreservation following initial consultation (p<0.001). The majority of patients self-identified as White (60%, 2107 patients). Compared to this group, Black or African American (OR 0.53, 95% CI 0.46-0.61; p<0.001), Hispanic or Latino (OR 0.56, 95% CI 0.42-0.75; p<0.001) and Unknown/Other/American Native patients (OR 0.39, 95% CI 0.32-0.48; p<0.001) were significantly less likely to continue treatment. Of those who underwent oocyte cryopreservation cycle(s), only 12% (425 patients) returned to thaw oocytes during the same timeframe, with 293 (69%) and 132 women (31%) utilizing partner and donor sperm, respectively. Ethnicity also was significantly associated with the decision to thaw previously cryopreserved oocytes (p<0.001). White patients comprised the majority, 66%, (281) of this group. Compared to this group, Black or African American (OR 0.67, 95% CI 0.48-0.92; p=0.02),) and Unknown/Other/American Native (OR 0.11, 95% CI 0.03-0.45; p<0.01), were significantly less likely to thaw their cryopreserved eggs. Additionally, ethnicity was significantly associated with source of sperm, with Black or African American women utilizing donor sperm significantly more than partner sperm (p=0.014). The decision to pursue fertility preservation, utilization of cryopreserved autologous oocytes, and choice of sperm source in subsequent thawed cycles is significantly associated with ethnicity. This may reflect socioeconomic barriers which disproportionately affect fertility treatment for certain ethnic minority groups." @default.
