- W4387410462 abstract "The oocyte is the primary locus of reproductive aging in women, but donor oocyte and PGT-A outcomes suggest degradation of endometrial function in older women. We evaluated the effect of age on endometrial assays in women receiving embryos from donated oocytes. Endometrial assays included the Endometrial Receptivity Assay (ERA) and Receptiva (for BCL-6 and cd138). This retrospective observational study included all donor egg recipients at NYU Langone Reproductive Specialists of New York from 2018 to 2022, when prep cycles with endometrial biopsy were offered to all patients undertaking DE treatment. The independent variable was age, and dependent variables were ERA and Receptiva results. Relationships between endometrial test results and pregnancy outcomes also were assessed. Patients with anatomical uterine anomalies were excluded. Univariate analysis examined the association of age at time of endometrial biopsy and ERA and Receptiva results. Multivariate logistic regression assessed the relationship between these assays and pregnancy outcome, controlling for confounding variables BMI and blood pressure. The model calculated odds ratios (OR) with corresponding 95% confidence intervals (CIs) and p values. 138 prospective DE recipients underwent endometrial biopsy during prep cycles. 22 were excluded because of missing information and 8 were excluded because of co-existing anatomical uterine anomalies. Mean age was 43.45 years old (range: 30-55, SD: 4.67, SE: 0.449) and mean BMI was 29.08 (range: 15.73-46.27, SD: 6.74, SE: 0.048). 57% of first-time embryo transfers resulted in pregnancy, and 36% resulted in live birth. Age significantly increased the odds of abnormal ERA (OR, 1.096; 95%CI, 1.01-1.20; p = 0.033). Implantation (OR, 1.30; 95% CI, 0.55 to 3.10; p = 0.39), miscarriage and live birth rates (OR, 1.506; 95%CI, 0.6307 to 3.642; p = 0.5104) did not differ between those with abnormal vs. normal endometrial assays. The findings of this retrospective observational analysis reveal a statistically significant effect of age on endometrial function. Abnormal endometrial assay results did not significantly affect pregnancy rates, presumably because subsequent treatment abrogated the effects of these endometrial abnormalities on outcomes. Limitations of this study included its modest sample size and inability to track pregnancy outcomes before correction of endometrial abnormalities." @default.
