- W4387410598 abstract "During the pandemic, many couples had huge fears commuting for fertility treatments to high-risk metropolitan areas, to minimize being exposed to the virus. IVF using transport of non-cryopreserved oocytes/sperm from a satellite office, where no IVF lab existed, to the central lab location have been reported to be safe (1,2) but the transport of non-cryopreserved embryos from a central lab back to the satellite office has never been reported. This case presents the first live birth of a completely two-way Transport IVF using a portable incubator where the patient underwent both oocyte collection and embryo transfer in a satellite office, with no IVF lab, which is 1.5 h and 30 miles away from the central IVF lab. The protocol was approved by the NYS Department of Health. Following signed consent, a 30-year-old patient with a history of unexplained infertility and previously failed 4 IUIs had a telehealth consultation for IVF at our university-affiliated private clinic. Because of the high prevalence of the virus in Manhattan NYC, the patient agreed for a two-way Transport IVF. Following controlled ovarian superovulation, follicular aspiration of 11 large follicles, performed in a regular examination room at the satellite office, was performed under local anesthesia in the vaginal wall and the aspirates were collected in usual tubes, tightly capped, and placed in a heated block (37°C) within a G95 portable incubator, with a 315C PTC certified thermometer, that can maintain a stable heat for 6 h. The aspirate tubes were not examined for the presence of oocytes at the satellite location but immediately sealed and placed inside the portable incubator. The tubes contained HEPES buffer for pH control and heparin to prevent coagulation. The sperm sample was also produced at the satellite location, washed, and placed in the same incubator. The aspirate tubes and the washed sperm were shipped immediately to the central lab. 10 oocytes were collected, 6 were mature, leading to 3 good-quality blastocysts that were cryopreserved. The patient was prepared for a frozen embryo transfer (ET) cycle at the satellite location. On the day of the ET, 2 blastocysts were thawed and loaded in a transfer catheter in a unique way, following many successful attempts using mouse embryos, to prevent the embryos from being pulled in either direction within the catheter during transportation. The catheter containing the embryos was shipped to the satellite location, by an experienced embryologist using the portable incubator, where the embryo transfer occurred in the same examination room where the follicular aspiration happened. The patient got pregnant with a singleton pregnancy, and had a smooth pregnancy and delivery of a healthy baby. This novel approach alleviated the psychological, physical, and financial burdens including travel expenses, and loss of time from work, for a couple who wanted treatment at a location that had no IVF lab." @default.
