Identifying modifiable factors to help manage symptoms and improve outcomes is crucial. Recent research suggests that lateral hip muscle morphology (size and quality) can impact on optimal hip function and pain presentation. However, the association between hip muscle morphology and hip-related quality of life (QoL) in active young adults with hip/groin pain is unclear. The aim of this study was to investigate the association between the size and level of fatty infiltrate of lateral hip muscles with hip-related QoL measures in active young adults with hip/groin pain. Methods: In a cross-sectional cohort study design, 180 (37 female) football players with a self-reported history of >6months of non-time-loss hip/groin pain and a positive flexion-adduction-internal rotation test were included. Participants completed the International Hip Outcome Tool (iHOT-33) and Copenhagen Hip and Groin Outcome Score (HAGOS) questionnaires. The volume and muscle fatty infiltrate of gluteal muscles and tensor fascia latae were assessed using magnetic resonance imaging (MRI). Study aims were examined using linear models, controlling for body mass index and age, incorporating sex-specific interaction terms as appropriate. Results: Increased muscle volume in the gluteus medius, minimus, and tensor fascia latae (TFL) were positively associated with iHOT QoL scores. Gluteus minimus demonstrated weak relationships with iHOT symptoms (R2=0.09, p=0.006), iHOT total score (R2=0.08, p=0.007) and iHOT sport (R2=0.08, p=0.003). Similarly, TFL had showed weak relationships with iHOT symptoms (R2=0.07, p=0.015) and iHOT total score ( R2=0.07, p=0.020), while gluteus medius had a weak relationship only with the iHOT sport (R2=0.08, p=0.003) subscale. A positive weak association was observed between the volume of the gluteus minimus muscle and the HAGOS sport (R2= 0.04, p=0.031) subscale. There were no sex interaction effects observed for muscle size and all QoL measures. No association was found between muscle fatty infiltrate and hip-related QoL measures. Discussion: Among active young adults with hip/groin pain, an increase in muscle volume in the gluteus minimus, medius, and tensor fascia latae muscles may result in better QoL scores on the iHOT-33 questionnaire. Targeted rehabilitation which aims to improve size and quality of these lateral hip muscles could potentially improve hip-related QoL. However, there is a need for further research to explore the complex relationship between lateral hip muscle size, fatty infiltrate, and hip-related QoL measures to determine the most effective interventions for improving size and quality of the lateral him muscles in young adults with hip/groin pain. Impact/Application to the field: Lateral hip muscle volume, specifically the gluteus minimus, gluteus medius and tensor fascia latae muscles, may have a significant influence on the hip-related QoL of young adults with hip/groin pain. Further research addressing optimal methods of improving muscle morphology could improve treatment strategies and outcomes for this population.
