- W4387496672 abstract "Whether psychosis should be considered as a spectrum or as individual psychotic disorders that are discrete entities is currently unclear. This project examines whether there are genetic and phenotypic differences between different psychotic disorder diagnoses in UK Biobank. Participants from UK Biobank were categorised into psychotic disorder groups based on ICD-10 diagnoses from linked hospital records. Individuals with multiple diagnosis were categorised separately. Schizophrenia and intelligence polygenic risk scores (PRS) were calculated using PRS-CS and an ancestry adjustment was applied. Phenotype variables related to demographics, mood and functioning were selected. PRS and phenotypes were compared between each psychotic disorder group and schizophrenia using pairwise logistic regressions. A total of 2012 participants with at least one psychotic disorder diagnosis were included and these were categorised into schizophrenia (N=759), delusional disorder (N=297), acute and transient psychotic disorder (N=165), schizoaffective disorder (N=86), unspecified psychotic disorder (N=265), multiple diagnoses of psychotic disorder (N=219) and co-morbid diagnoses of schizoaffective disorder and schizophrenia (N=121). Compared to individuals with schizophrenia, a lower schizophrenia PRS was observed in those with diagnoses of delusional disorder (OR = 0.80, 95% CI = 0.69,0.92, p = 0.0025), acute and transient psychotic disorder (OR = 0.76, 95% CI=0.63,0.91, p = 0.0027) and unspecified psychotic disorder (OR = 0.73, 95% CI = 0.63,0.86, p = 8.55 × 10-5). Phenotypic differences were also found between psychotic disorder diagnoses; Those with schizophrenia were more likely to be male than those with delusional disorder or schizoaffective disorder. Individuals with acute and transient psychotic disorder, unspecified psychotic disorder, multiple diagnoses, schizoaffective disorder had higher rates of mood comorbidities than those with schizophrenia which in turn had higher rates of mood comorbidities than delusional disorder. Measures of better functioning such as current employment and cohabiting were higher in those with wider psychotic disorder diagnoses than with schizophrenia. We found evidence of genetic and phenotypic differences between individuals with schizophrenia and other psychotic disorder diagnoses in UK Biobank." @default.
