• W45747674 abstract "ZusammenfassungDie Verbesserung der Hämodynamik in Ruhe und unter Belastung ist eines der Hauptziele antiarrhythmischer Therapie. Dabei wird eine Normalisierung der Frequenz und die Wiederherstellung physiologischer Erregungsabläufe angestrebt. Bei bradykarden Rhythmusstörungen kann die Auswurfleistung des Herzens allein durch Frequenzanhebung gesteigert werden; eine physiologische, AV-sequentielle Stimulation mit/ohne Frequenzadaption unter Belastung ermöglicht eine individuelle Optimierung des Therapieeffektes. Bei ektopen und tachykarden Arrhythmien stehen medikamentöse Maßnahmen im Vordergrund. Im Einzelfall abzuwägen sind hämodynamische Vorteile der Regularisierung gegenüber kardiodepressiven Wirkungen der Antiarrhythmika. In der Gruppe der Lokalanästhetika entfalten vor allem die Chinidin-artigen Pharmaka, insbesondere Disopyramid, deutlich negativ inotrope Wirkungen, während Lidocain-artige Substanzen nur schwach kardiodepressiv sind. ß-Sympathikolytika wirken negativ chronotrop und inotrop, das Ausmaß dieser Effekte wird durch den Grad der sympathischen Stimulation des Herzens bestimmt, außerdem durch das Vorhandensein oder Fehlen intrinsischer sympathomimetischer Aktivität. Das Verhalten der Klasse-III-Antiarrhythmika ist uneinheitlich. Für einen Teil der Präparate (Amiodaron, Bretylium) wurde ein positiv inotroper Effekt nachgewiesen. Die herzwirksamen Kalziumantagonisten entfalten dosisabhängige negativ inotrope Wirkungen. Diese werden durch die arterielle Vasodilatation teilweise antagonisiert, so daß die Belastungshämodynamik weniger eingeschränkt wird als durch ß-Sympathikolytika. Bei Einsatz von Antiarrhythmikakombinationen ist mit additiven negativ inotropen Effekten zu rechnen. Hämodyna- misch ungünstig erscheinen Kombinationen der Chinidin-artigen Pharmaka mit ß-Sympathikolytika und- besonders bei kombinierter i. v. Anwendung - von ß-Sympathikolytika mit antiarrhythmisch wirksamen Kalziumantagonisten.Die Unterschiede der hämodynamischen Wirkungen sind von praktischer Bedeutung für die Auswahl der Therapiemaßnahmen, besonders bei Myokardinsuffizienz. In der Neu- und Weiterentwicklung von Antiarrhythmika sind geringe hämodynamische Nebenwirkungen, verbunden mit hoher Wirkstärke, wesentliche Ziele. Beispiele hierfür sind das neue Lokalanästhetikum Barucainid (Klasse Ib) und die Klasse-III-Substanz d-Sotalol." @default.
