- W46094020 abstract "2141 Objectives Gaucher9s disease (GD) is a hereditary lysosomal storage disorder. Infiltration of the bone marrow by Gaucher9s cells can be evaluate by MR or by 99mTc-Sestamibi scintigraphy. Since [18F]FDG has substituted 99mTc-Sestamibi as a tracer of cellularity/metabolism in several condition, we evaluated the performance of [18F]FDG-PET/CT in GD. Methods We enrolled 7 pts with type-1 GD (mean age 33 yr); 2 pts had repeated scans whilst on enzyme replacement therapy (ERT). Two pts had never received ERT, while 5 pts were on ERT when performing the scan. [18F]FDG (3.7 MBq/kg) was injected i.v., and images recorded 60 min later. A semiquantitative score based on extension and intensity of [18F]FDG uptake in the bone marrow was defined, as described for 99mTc-Sestamibi scan (0 = no uptake; 8 = maximum uptake). The CT component of the scan served to evaluate liver/spleen volumes. Results [18F]FDG-PET/CT showed some degree of bone marrow disease in all pts: score 2 in two pts, score 4 in three, score 6 in one, score 7 in one, and score 8 in two ERT-naive pts. The [18F]FDG-PET score was highly correlated with the clinical severity score index (SSI). In ERT-treated pts, the score was 2 and 7 at baseline, worsen to score 4 in case of ERT discontinuation and decrease to 6 in presence of ERT response. Liver/spleen volumes were stable in one case and declined in the other pt responding to ERT, consistently with improved hamatologic parameters and chitotriosidase levels. Conclusions [18F]FDG-PET/CT was extremely sensitive in defining bone marrow infiltration by Gaucher cells, providing a PET score highly correlated with the SSI. CT data were useful to evaluate liver/spleen volumes and their variation in response to ERT. These preliminary data suggest that [18F]FDG-PET/CT is a valid alternative to 99mTc-Sestamibi scintigraphy for assessing bone marrow involvement in GD" @default.
