It has to be noted from the outset, however, that any attempt of theoretical generalization from empirical data is immediately vulnerable to critique because of the unsettled, yet crucial epistemological issues. The most significant of such questions is the very notion of schizophrenia (a strange reservation to make nearly 100 years after this concept was created [1]). Conceptual limitations, if not plain incoherence, of the polythetic operational approach to psychiatric classification [2, 3] have transformed schizophrenia into a diagnosis that is essentially made by exclusion. It is more defined by whatit is notthan by what it actuallyis [4]. Its psychopathological essence is progressively eluding clinical community and is alarmingly ignored in the empirical research [5]. Such psychopathological void is permissive ofany theoretical position, no matter how remote from clinical and phenomenological realities. In other words, in the absence of a clear phenomenological concept of schizophrenia, it is nearly impossible to entertain a meaningful discussion of its boundaries, pathogenic mechanisms or potential etiological heterogeneity." @default.
