Concern over both the quality of life (QOL) of the individual and the allocation of health care resources is a major reason for the large amount of research in this area. The discussion in this chapter has the broad goal of examining depression and chronic illness in relation to the end of life. Depression and physical illness are analyzed as antecedents of treatment preferences near the end of life, explicitly treatment whose purpose is to extend life. Inasmuch as little is known about how such individual characteristics determine actual treatment received, the outcomes to be examined are usually expectations or preferences for treatment or, alternatively, wishes for length of life given hypothetical conditions under which life would exist. The overall guiding hypothesis whose tenability will be examined in a critical literature and conceptual review is that the wish to live a shorter time is partially explainable on the basis of depression (and other negative aspects of life), but that such a relationship is also conditioned in the opposite direction by the positive qualities of the person’s life. Valuation of life (VOL) is offered as a construct proposed to mediate the relationship between negative and positive aspects of mental health and end-of-life wishes and behaviors. The first section of this chapter describes briefly the concept of health-related quality of life (HRQOL) and the place given depression in the operationalization of HRQOL. The next section sketches my conception of QOL and mental health, and provides definitions of several important constructs.
