Patients with life-limiting illnesses report that communication and affirming personal relationships as death grows near, aids in achieving ‘‘life completion.’’ Patients have also identified love, gratitude, and forgiveness as important relational expressions. These expressions have not been systematically measured in patients with serious illness. Research objectives. Psychometrically evaluate the ARCom, a new screening instrument that assesses important personal relationships (eg, self, others, God/higher power) and whether individuals have communicated expressions of love, gratitude, and forgiveness. Methods. Face-to-face interviews were conducted with a convenience sample of 125 adult patients with advanced cancer in outpatient oncology centers. The ARCom questionnaire consists of 13 item pairs. Participants scored the 13-item pairs and indicated the item content as ‘‘important’’ and the extent to which it had been completely addressed. One point was scored for each item pair that was deemed important but not resolved. High scores indicate a greater number of unresolved relational expressions. Outcomemeasures included life completion, relational conflict, and symptom assessment subscales. Psychometric analyses included item-total correlations, Chronbach reliability, and convergent and discriminant validity. Statistical tests included correlation, t-tests, and analysis of variance. Results. Mean age was 61 years (range 23e90). Fifty percent of the sample reported at least one unresolved relational expression with 30% reporting more than one. Chronbach reliability for the total scale was 0.68. Significant positive correlations were observed between the total score, negative mood states, and interpersonal conflict and negative correlations with life completion and psychological symptom scales. Conclusion. One in two advanced cancer patients experienced unresolved relational expressions. This clinically useful screening instrument will provide palliative care clinicians the ability to target specific relational and communication needs of patients with serious illness. Implications for research, policy, or practice. Helping patients affirm relationships and identify hurts may facilitate life completion. Explicit assessment of personal relationships and communication may lead to powerful interventions as time becomes paramount." @default.
