- W50871722 abstract "Costa Rican women have been participating in cervical cancer research since 1985, when the USA National Cancer Institute (INC) started financing studies to determine the causes of cervical cancer. Most of those studies were conducted in one of Costa Rica’s poorest rural regions with a high incidence of HPV infection, and involved thousands of healthy young women. Undoubtedly the results of these studies are of high scientific and economic value, but they have not been free of controversy. Drawing on archival information (including legislative reports, academic and newspaper articles) and the testimonies of local experts and journalists, this chapter describes the complexities surrounding longitudinal clinical research projects, and the conflicts of interest and ethical violations that have been uncovered during the 20 years of cervical cancer research in this region. This case study illustrates the weaknesses of the regulatory framework and its enforcement, the strategies used by the promoters of clinical trials to foster clinical research and bypass existing regulations, the efforts of legislators and human rights activists to protect the rights of the participants and public health resources, and the conflicts of interest that have permeated the highest echelons of the Ministry of Health and the Costa Rican Social Security Institute. Despite ethical concerns in the informed consent form and in the recruitment of participants, some of these studies were approved by three research ethics committees.
