In the past 15 years four theories have been predominantly discussed that relate the onset of irreversibility to changes in the cells’ biochemical state: (i) a critical energy loss; (ii) a critical accumulation of cellular calcium; (iii) deleterious effects of accumulation of long-chain acyl compounds; or (iv) the effects of free radical formation. These theories do not represent mutually exclusive alternatives, each of them may accentuate a certain part of the pathophysiological process leading to irreversible myocardial injury.
