- W55086280 abstract "Many issues in the treatment of epilepsy are sex-specific, and several antiepileptic drugs (AEDs) have gender-specific side-effects. These effects may have an impact on several aspects of life, including pubertal development, fertility, sexuality, self-esteem through cosmetic changes, and (at an older age) menopause and bone health. Such side-effects should be considered when starting treatment with antiepileptic drugs.Both women and men with epilepsy are at higher risk for sexual dysfunction than are persons without epilepsy. Although the reasons for this risk are multifactorial, the impact of certain AEDs appears to be relevant, since the use of enzyme-inducing AEDs (EIAEDs) seems to increase the frequency of sexual problems in both genders. EIAEDs may affect reproductive endocrine function by reducing the free fraction of steroid hormones, primarily by increasing the synthesis of sex-hormone binding globulin. The use of valproate (VPA), however, has been associated with increased serum androgen levels in both sexes. In women, VPA also seems to increase the prevalence of menstrual disorders and polycystic ovaries. With the exception of lamotrigine, which does not seem to have effects on reproductive endocrine function, newer AEDs have only rarely been investigated for endocrine effects.Several AEDs are associated with disorders of bone metabolism and osteoporosis. This condition is of special importance for postmenopausal women with epilepsy, who are especially at risk owing to low estrogen levels.Increasing the understanding and awareness of gender-specific problems and gender-specific side-effects of AEDs among both patients and health care professionals is essential to prevent and reduce many of these AED-related problems and to improve the quality of AED treatment in both women and men." @default.
