- W56936618 abstract "This chapter discusses issues pertinent to the female gender in colorectal cancer and inflammatory bowel disease (IBD). In the case of colorectal carcinoma, there appears to be a slight disparity in incidence and site distribution in the colon between men and women. Although hormonal factors appear to have some effect on the development of colorectal neoplasms, the influence is weak. Women tend to have limited knowledge regarding their own risks of cancer. The treatment of IBD in the face of pregnancy can be difficult. Fertility is preserved in patients with quiescent IBD, but active disease diminishes fertility and increases fetal complication rates. Disease activity itself does not seem to be influenced by the hormonal changes of pregnancy. Medications for IBD are generally safe during pregnancy and nursing (with a few exceptions) and may need to be used judiciously during pregnancy. Fertility is diminished after restorative proctocolectomy, although pregnancy in patients with an ileal pouch is safe. This chapter discusses anatomy and physiology of fetal incontinence, its causes, history and examination, appropriate tests, its medical management, the concept of biofeedback, and consideration of surgery. Fecal incontinence tends to occur more commonly in the female gender. Obstetric trauma is an important cause of fecal incontinence that uniquely affects women. The overlapping sphincteroplasty is the procedure of choice, but functional improvements after surgery may be modest and continence may worsen over time." @default.
