- W591022012 abstract "目的:痔核の低侵襲性治療法として，硫酸アルミニウムカリウムとタンニン酸を用いたジオン注射硬化療法（ALTA）やProcedure for Prolapse and Hemorrhoids（PPH）は治療の選択肢を拡げた．その有用性を検討した．方法:2006年1月から2007年12月まで結紮切除術（LE）331例，ALTA 565例，PPH 73例を施行した．LEとALTAは2-3度の痔核脱肛を，PPHは全周性や粘膜脱を伴う脱肛を適応とし，成績を比較した．結果:LEは入院6.8±2.0日（m±SD）で再発はなかった．ALTA単独療法は日帰りで，注入量は痔核当たり7.1±2.1mLで狭窄や潰瘍はなく，痔核縮小・消退を543例（96.1％）に得られた．PPHは入院4.2±1.5日で疼痛や合併症なく72例（98.6％）で有効であった．考察:ALTAでは合併症の回避を重視し，重篤な合併症なく多数例で十分な効果を得た．PPH03は止血能が増し低侵襲性が向上したが，先進医療で自己負担額が高い．結論:LEは適応が広く再発はないが，術後痛を伴い入院が必要である．ALTAは外来治療が可能で疼痛や合併症も少ない．PPHは入院期間が短く疼痛も少ない．低侵襲性治療法は合併症の発生に留意して行えば，有用な方法である．" @default.
