- W609097586 abstract "Mother-to-child transmission ofhuman immunodeficiency virus type 1 (HIV:l) has bec orne one of the fastest growing aspects of the AIDS pandemic. The objectives of this work based on pregnant women are: a) to study HIV -1 genetic diversity; b) to characterize HIV -specific CTL responses; c) To analyze the genetic evolution of HIV strains; and d) to assess the impact of antiretroviral therapy on both CTL responses and HIV viral population. Our study of HIV genetic diversity has shown that 42.7% of patients were infected with non-clade B· viruses. A total of 97.7% of non-clade B viruses were found in African women and, in all cases, clade identity was consistent with variants circulating in patient's countries of origin. We successfully demonstrated that dates of arrivaI into Canada of patients infected with nonc1ade B HIV correspond with the migration of refugees from African HIV -endemic areas. We then made a longitudinal characterization of HIV -specific CTL effectors responses and assessed the modulation of this arm of cellular immunity by antiretroviral therapy in HIV infected women. First, we clearly established that HIV -specific CTL responses become progressively more prevalent as HIV disease progresses. ART treatment was associated with a decline in CTL responses. No significant variations were observed between first, second, and third trimesters, or between consecutive pregnancies. Then, we found strongest CTL responses against Gag, Env, Pol and gp120. Finally, the extent of variation in the hierarchy of antigenic recognition observed between consecutive pregnancies was correlated with the duration of the inter-pregnancy interval. Finally, our assessment of the diversity of env gene showed a longitudinal evolution of HIV viral populations throughout pregnancy and across the intre-pregnancy interval, and this evolution seemed more important when viral replication is controlled. The longer interpregnancy intervals were associated with largest magnitude of Env V3 domain diversification. We also found lowest diversity at the third trimester for all Env domains" @default.
