• W635374002 abstract "Heart Failure (HF) is a serious public health problem, characterized by high rates of incidence and prevalence worldwide. HF is responsible for the increased rate of hospitalisation, rehospitalisation and mortality. With high rates of hospitalisation and rehospitalisation, costs become higher for the health systems. Home Telemonitoring (HTM) can help the HF patients and also the health systems to fight against the high rates of hospitalisation and, consequently, the high expenditures. The present work has the main aim to prove the potencial of HTM in reducing costs, rates of hospitalisation/rehospitalisation due to decompensation of this pathology. The study will compare the costs of all hospitalisations due to exacerbation of HF occurred in 2010 at the Hospital Pedro Hispano (HPH) with the cost of implementation and use of a HTM system to apply these same patients. It will be used some studies already carried out about the HTM in the HF as the basis of their ability to reduce high rates of hospitalisation associated with HF. Studies support the potential of the HTM to reduce the rate of hospital admission associated with chronic HF from 1,68% to 68,3%. The total cost of 410 admissions occurred in HPH in 2010 was € 1.421.084,48. According to estimates provided by two companies that sell telemonitoring solutions in Portugal, the annual cost of implementation and use of HTM can vary between € 966.386,40 (ISAIntellicare) and € 1.010.420,4 (TCare). The costs obtained for the implementation and use of HTM in patients with chronic HF proved to be less than the cost of hospitalisations due to exacerbation of this disease. The reference studies point to the potential ability of the HTM to reduce hospitalisation rates. The reduction of 21% was the result observed with greater frequency. The purchase of any product requires the amortization of its cost in a given time period. The amortization of the amounts spent with HTM by the method of constant shares would be achieved with a reduction in hospitalisation rates of 9,41% per year for 3 years. As the result observed more frequently in reference studies was a reduction in the rate of admission of 21% (more than 9,41%) it can be expected that the National Health System can spend less with chronic HF through TMD.
