- W63808727 abstract "Background: Left atrial volume (LA) is assuming increasing importance as a biomarker for diastolic dysfunction and has prognostic significance. A number of approaches have been used for LA measurements, and difficulties in reproducing imaging planes may limit reproducibility on repeat testing. We sought the variation of sequential echo for following LA size using 2D and real-time 3D echocardiography (RT3D). Because 3D echo has previously been shown to correlate well with magnetic resonance imaging, we used this as a reference standard for the other techniques. Methods: Patients (n=35, 24 males, age 50±17) presenting for evaluation of LA were studied with 2D and RT3D. Test-retest variation was performed by a complete re-study by a separate sonographer within 1 hour without alteration of hemodynamics or therapy. A subgroup of 15 patients (11 males, age 42±18) was studied for inter- and intra-observer variation. LA volumes were calculated by the methods of M-mode, Simpson’s biplane, biplane area-length and RT3D. Results:The average LA volume was 58±27 by RT3D, 30±18 by M-mode (p<0.01), 61±26 by area-length (p=0.67) and 60±27 by Simpson’s biplane (p=0.47). The table summarizes correlation and differences from the mean between sequential (test-retest), inter- and intra-observer measurements. 2D approaches were superior toM-mode, with both 2D approached being similar. There was slightly wider scatter in the mean (DM) of LA volumes with 2DE than RT3D.
