- W64802883 abstract "The relationship between lead and gout has long been recognized (1). Lead has been responsible for several epidemics of gout (2–4) and may cause an interstitial nephropathy (5,6) characterized by severe uric acid underexcretion (7). In the past decade, and despite no direct apparent exposure to lead, several reports have documented increased lead body stores in patients with gout and renal insufficiency (8–11). Renal failure by itself could not account for the increased body lead stores since non-gout patients with chronic renal disease had normal urinary lead excretion rates (8–11). In contrast, other authors were unable to demonstrate that lead was important in the pathogenesis of renal failure in patients with gout (12). The present study was undertaken to determine: (a) body lead stores in patients with gout at our institution, (b) whether an increased body lead content could be related to the derangement in purine metabolism characteristic of gout, and (c) if renal function and purine metabolism are modified following chelation therapy and normalization of increased mobilizable lead." @default.
