Inthis study, we aimed to evaluate the results of 50-gram glucose challenge test in patients with orwithout risk factors of diabetes and determine acut-off level. At 24-28th weeks of pregnancy, a 1-hour 50-gram glucose challenge test was performedin all patients followed by a 3-hour 100-g oral glucosetolerance test in patients with a glucose levelof 130 mg/dL or above. Patients having at least 2values at or above the cut-off were diagnosed tohave gestational diabetes mellitus. Patients havingonly one value at or above the cut-off underwent asecond oral glucose tolerance test 4 weeks later. Inthe second oral glucose tolerance test, patientshaving at least 2 values at or above the cut-off wereaccepted to have gestational diabetes mellitus andpatients having only one value at or above the cutoffwere accepted to have glucose intolerance. Exerciseand diet therapy were initiated for womendiagnosed to have either glucose intolerance orgestational diabetes mellitus. These patients werefollowed every two weeks to obtain fasting andpost-prandial glucose levels. The aimed levels offasting and post-prandial glucose were 105 mg/dLand 120-140 mg/dL, respectively. In cases of failedglysemic control despite exercise and diet therapy,patients were hospitalized for insulin therapy. 320pregnant women were included in the study. Ofthese patients, 171 had no risk factors for gestationaldiabetes mellitus while 149 had one or morerisk factors. In the whole study group, 128 patients(40.0%) had a positive 50-gram glucose challengetest and 20 (6.2%) had a positive oral glucose tolerancetest. While 58 (33.9%) of patients withoutany risk factors had a positive 50-gram glucosechallenge test, 70 (47.0%) of patients having riskfactors had a positive 50-gram glucose challengetest. In addition, While 4.1% of patients withoutany risk factors were diagnosed to have gestationaldiabetes mellitus, 8.7% of patients having riskfactors were diagnosed to have gestational diabetesmellitus. In conclusion, we suppose that a singlecut-off value for 50-gram glucose challenge test isnot reasonable. We also conclude that screeningtest for gestational diabetes mellitus should beperformed for all pregnant women at 24-28th gestationalweeks and cut-off value should be set to 140mg/dL for patients having risk factors while itshould be 150 mg/dL for patients without any riskfactors." @default.
