Factors to be considered are the extent and viability of the myocardium supplied by the coronary artery undergoing occlusion, the extent of collateral circulation to the coronary bed distal to the occlusion and the duration of coronary occlusion. Reversible ST-segment elevation is generally considered to reflect transmural myocardial ischemia and ST depression to reflect non-transmural or subendocardial ischemia [1–2]. Hemodynamic and angiographic observations in humans and animal models of coronary artery spasm have suggested that ST-segment elevation reflects either total coronary occlusion in the absence of developed collateral circulation of subtotal occlusion with or without an increase in myocardial oxygen demand [3–6]." @default.
