Las infecciones de piel y partes blandas causadas por Staphylococcus aureus meticilino resistente de la comunidad (SAMRco) forman parte de las enfermedades emergentes, y su incidencia en nuestro medio es desconocida. Como objetivos planteamosconocer nuestra incidencia de infecciones por SAMRco, formas clinicas mas comunes, sensibilidad antibiotica y caracterizacion molecular de los aislamientos.Material y metodos. Realizamos un estudio prospectivo, en el que se detecto en 30 casos SAMRco como agente causal. En 9 cepas seleccionadas al azar se realizo estudio molecular y genetico. La duracion del estudio fue de 15 meses.Resultados. Se confirmo por estudio microbiologico la infeccion por SAMRco en 30 pacientes. En los 9 casos seleccionados al azar, en quienes se llevo a cabo el estudio molecular y genetico, se demostro que todas las cepas portaban la leucocidina de Panton Valentine, el gen mecA, y el casete cromosomico SSC tipo IV. Por PFGE se agruparon endos tipos clonales; uno de ellos, constituido por 4/9 aislamientos, es una variante del clon prevalente previamente descripto en la Argentina (CA-PFGE A, caracterizado como SARM-IV-ST5-spa t311-LPV+).Conclusiones. En pacientes con lesiones sospechosas encontramos una frecuencia de infecciones por SAMRco del 81%. Todos los SAMRco fueron sensibles a TMS-SMX. La resistencia a clindamicina se observo en el 21% de los casos. Son necesarios estudios adecuados de incidencia en nuestro pais, asi como tambien el mayor conocimiento de las caracteristicas de las cepas de SAMRco en nuestra zona geografica(Dermatol. Soft tissue and skin infections caused by Community-Acquired Methicillin- Resistant Staphylococcus aureus (caMRSA) are part of an emerging group of diseases, and their real incidence in our country is unknown.Methods. A prospective study that included 30 patients with Community-Acquired MRSA was made. In 9 randomized cases genetic and molecular studies were performed.Results. Infection by caMRSA was confirmed in 30 patients by microbiology testing. Leucocidin Panton valentine, mecA gene and SSC IV was found in 9 randomized cases by molecular and genetic studies. By PFGE they gathered in crowds in two clonal types; one of them, constituted by 4/9 isolations it is a variant of the prevalent clone previously described in Argentina (CA-PFGE A, characterized MRSA-IV-ST5-spa t311-LPV+)Conclusions. In patients with high clinical suspicion we found 81% of the cases were caused by caMRSA. All the caMRSA were sensitive to TMS-SMX. Resistance to clindamycine was found in 21% of the cases. More suitable studies of incidence are necessary in our country, as well as further knowledge about the characteristics of caMRSA in our geographical zone.(Dermatol.
