Fase transversal (visita inicial) de un estudio de intervención comunitario en atención primaria (AP). La población diana del estudio está constituida por todos los asmáticos, pertenecientes al área 2 de Madrid y registrados en 10 centros de salud, mayor de 14 años y con síntomas de su enfermedad en el último año. La selección de los pacientes para el estudio fue sistemática (no probabilística) con reclutamiento consecutivo de los asmáticos que acudían y/o citados entre los registrados. Tasa de participación: 96,6% de los registrados. Se recogieron variables sociodemográficas, clínicas y funcionales, así como estudio alérgico, antecedentes familiares de asma y/o atopia y tipo de régimen terapéutico. Se estudiaron 650 pacientes con una media de edad de 45 ± 28,1 años y FEV,% de 87,2% ± 26; 238 (36,7%) eran varones y 412 (63,3%) mujeres. La media de evolución de la enfermedad fue de 16 ± 14,6 años, con edad de comienzo de 26 ± 20 años. El 34% tenía antecedentes familiares de asma y el 21% de atopia; el 50% tenía como factor asociado una rinitis alérgica, siendo los factores desencadenantes más importantes las infecciones respiratorias (62,4%), seguidas del polen (25,3%) y la exposición al humo del tabaco (20%). El 9,1% eran fumadores activos, siendo ex fumadores el 21,6%. El 64% de los pacientes utilizaban los fármacos betaadrenérgicos de corta duración a demanda, a diferencia del empleo diario tanto para los beta 2 de larga duración (83,2%) como para los esferoides inhalados (77%). El estudio establecido según grados de severidad determinó diferencias significativas en relación a la edad de los pacientes, edad de comienzo y predominio de asma perenne (siendo mayores en el grupo de asma grave). a) La mayoría de los asmáticos sintomáticos son leves, y b) se debe incidir en una pauta farmacológica correcta ajustada a un diseño terapéutico racional y escalonado, según el grado de severidad de la enfermedad. To determine the social, demographic, clinical and lung function characteristics of the population of asthmatics living in Health Care District 2 of Madrid. The target population consisted of all asthmatics in District 2 of Madrid who were registered at the public health clinics, were oider than 14 and had experienced asthma symptoms within the past year. We recorded social, demographic, clinical, and lung function variables as well as results of allergy studies, family histories of asthma and/or atopy and type of treatment. Six hundred fifty patients were enrolled, 238 men (36.7%) and 412 women (63.3%). Mean age was 45 ± 28.1 yr, mean FEV 1 % was 87.2 ± 26%. Mean course ofdisease was 16 ± 14.6 yr and age of onset was 26 ± 20 yr. Family history of asthma was present in 34% of cases and a family history of atopy was reported by 21%. Half had allergic rhinitis as an associated factor, with respiratory infections (62.4%) being the most common trigger, followed by pollen (25.3%) and exposure to tobáceo smoke (20%). Active smokers accounted for 9.1% of the group, and ex-smokers for 21.6%. Short-term beta-adrenergic drugs on demand were used by 64% of patients, whereas 83.2% reported daily and long-term use of beta-2 drugs and 77% used inhaled steroids.
