In both patients, the carcinoidswere associated with extraintestinal neo-plasms. Our findings support the associa-tion of carcinoids with other neoplasticconditions.Case 1A 43-year-old woman underwent a la-paroscopic left ovarian cystectomy forwhat proved to be an incompletely ex-cised invasive mucinous carcinoma with asmall focus of endometriosis. She sub-sequently underwent total abdominalhysterectomy and bilateral salpingo-oophorectomy, peritoneal cytology and alymphadenectomy. At laparotomy, multi-ple nodules along an extensive length ofileum were incidentally noted. Examina-tion of a frozen section revealed a carci-noid neoplasm. A small bowel resectionwas performed, encompassing the af-fected portion of the bowel and an adja-cent enlarged and hard lymph node in thesmall bowel mesentery (Fig. 1). The finalpathology revealed more than 40 carci-noids in mucosa, submucosa, muscularispropria and subserosa of the small bowel(Fig. 1). The histologic pattern was pre-dominantly insular; however, rare trabec-ular and glandular patterns were alsoseen. Lymphatic invasion was identified inthe small bowel wall and the surroundingfat. The proximal and distal margins werenegative for neoplasm, and 9 of 13 mes-enteric lymph nodes were positive formetastatic carcinoid. The appendix wasgrossly and histologically unremarkable.The patient was disease-free on follow-upalmost 3 years after her operation.Case 2A 77-year-old woman presented with a his-tory of multiple emergency visits and ad-missions to the hospital for recurrent smallbowel obstruction over a 2-year period. Asmall bowel follow-through examinationwas normal. Abdominal ultrasound re-vealed a multilobulated terminal ilealmesenteric mass measuring 3 × 3.2 cm,likely representing a lymph node that wasvery vascular. A computed tomographyscan of the abdomen and pelvis withcontrast showed no evidence of smallbowel obstruction, with focal cecal wallthickening and 2 large mesenteric massesmeasuring 3.5 ×3.4 cm and 1.7 1.9 cm.Colonoscopy revealed a sigmoid tubularadenoma, which was removed. Ex-ploratory laparotomy findings included anormal cecum and multiple ileal lesions" @default.
