L’instabilité peut être soit idiopathique, soit post-traumatique. L’instabilité post-traumatique semble la plus fréquente. Le diagnostic est souvent retardé car méconnu. Il s’agit souvent d’une douleur externe pseudo-méniscale associée à une sensation d’instabilité lors de la marche. La proximité du nerf fibulaire commun peut être à l’origine de lésions nerveuses par étirement. Le diagnostic de la luxation est essentiellement clinique. Les radiographies sont difficiles d’interprétation et les coupes TDM permettent de situer avec plus de précision la position de la tête de la fibula par rapport à la diaphyse tibiale. L’obliquité de l’interligne articulaire et l’hyperlaxité ligamentaire sont les seuls facteurs favorisants reconnus. Le mécanisme luxant initial est soit un choc direct sur l’articulation, soit un mécanisme indirect de torsion combinée de la cheville et du genou. L’arthrodèse isolée et la résection de la tête de la fibula ne sont plus utilisées. Diverses ligamentoplasties ont été proposées mais exposent à une détente secondaire. L’intervention de référence semble être l’arthrodèse avec résection sous jacente de la fibula selon le principe de Kapandji-Sauvé. Nous proposons une mise au point sur cette pathologie à propos d’un cas clinique associée à la revue de la littérature. Instability of the upper tibia-fibula junction is an uncommon finding. Only 150 cases of acute or recurrent dislocation have been published in the literature. Instability can be idiopathic or secondary to a traumatic event. Posttraumatic instability appears to be more frequent. The diagnosis, often missed, may be established late. Patients generally complain of lateral meniscus-like pain associated with subjective instability when walking. The common fibular nerve lies in the proximity and may be stretched, leading to nervous lesions. The diagnosis of dislocation is generally clinical. It may be difficult to interpret plain x-rays. More precision can be obtained with computed tomographic slices which can be used to establish the position of the fibular head in relation to the tibial shaft. The only known favoring factors are an oblique joint line and ligament hyperlaxitiy. The initial dislocation is caused by a direct impact on the joint or via an indirect mechanism due to combined torsion of the ankle and the knee. Most surgeons prefer isolated arthrodesis plus resection of the fibular head. Ligamentoplasty may be proposed but with the risk of secondary loosening. The standard treatment would be arthrodesis with resection of the underlying fibular using the Kapandji-Sauvé method. We present an update on this condition and present a clinical case and a review of the literature." @default.
