- W800807304 abstract "Since they allow critically ill and injured patients to be transported early, Critical Care Air Transport Teams (CCATTs) are one of modern casualty care's integral components. Due to transport ventilator limitations and potential further in flight respiratory deterioration, CCATTs sometimes find aeromedical evacuation of patients with significant pulmonary impairment beyond their scope. To facilitate transporting these patients from the combat theater, the Acute Lung Rescue Team (ALRT) was developed. Study methodology consisted of reviewing all material on critical patients transported out of the combat theater between November 2005 and March 2007 found in the United States Army Institute of Surgical Research Joint Theater Trauma Registry and the United States TRANSCOM Regulation and Command/Control Evacuation System databases. There was comparison of CCATT and ALRT patients after the abstracting of patient clinical history, diagnosis, and demographics. During the study period, there was activation of the ALRT for 11 patients. As a result of these activations, there was transportation of five patients. Eighty-two percent of these requests were made possible through trauma-related diagnosis. Of all ventilator transports, ALRT missions comprised 1%, and of all critical patient movements, ALRT missions comprised 0.6%. For CCATT patients (p = 0.0005) average FIO sub 2 was 0.53 ± 0.14 and for ALRT patients, it was 0.92 ± 0.11. The CCATT group required a mean positive end expiratory pressure of 6.5 cm H sub 2 0 ± 2.4 cm H sub 2 0 in comparison to ALRT patients, who required a mean positive end expiratory pressure of 19.0 cm H sub 2 0 ± 2.2 cm H sub 2 0 (p = 0.002). It is uncommon for the severity of combat theater lung injury to exceed CCATT transport capability. Significantly higher FIO sub 2 and positive end expiratory pressure was seen in ALRT activated patients than in CCATT transported patients. ALRT transport consideration may have been futile for one quarter of the patients for whom it was considered, since they died before a team could be launched. Experienced, equipped specialty teams can successfully transport patients with even severe acute respiratory distress syndrome." @default.
