- W82196327 abstract "Publisher Summary This chapter provides an overview of the studies of the multiple molecular forms of bioactive parathyroid hormone and parathyroid hormone-like substances. Recent studies have emphasized the complexity of the biosynthesis and the processing of parathyroid hormone within the parathyroid gland, which includes not only multiple cleavage steps but also, at least to some extent, modification of the peptide chain by phosphorylation. Once released by the parathyroid cell, the extra glandular metabolism of the gland is equally complex and may be altered in disease states. Thus, in hyperparathyroidism with normal renal function, the major circulating bioactive form of the hormone is similar or identical to the major glandular form, consistent with glandular hypersecretion. However, in end-stage renal disease, in addition to hypersecretion of the major glandular form of PTH, altered metabolism of the hormone appears to occur and contribute smaller material to the total complement of circulating bioactive hormone. In examining circulating bioactive forms of PTH, a discordance between bioactive and immunoreactive levels of hormone or hormone like entities has been found in selected cases of both pseudohypoparathyroidism and hypercalcemia associated with malignancy. At the target tissue level, in vivo studies have suggested that the inter action of PTH with target cells is as complex as intra- and extraglandular metabolism. Thus, binding reactions to high and low capacity sites on discrete membrane regions of the same or different cells, and to sites demonstrating specificity for different sequential regions of the molecule, can be demonstrated." @default.
