- W82396021 abstract "急性期の脳卒中患者のトリアージと，特にt-PA療法の適応のある脳梗塞患者のトリアージを目的にして，救急隊と脳卒中プレホスピタルスケール（TOPSPIN：TOYOTA prehospital stroke scale for t-PA intravenous therapy-経静脈的t-PA療法のためのトヨタ脳卒中プレホスピタルスケール-）を導入した．今回，このTOPSPINに連動し，特に超急性期t-PA療法に対する院内体制の迅速化，標準化を計るため，（1）救急外来で脳卒中の鑑別と，特に脳梗塞ではt-PA療法の適否決定のための諸検査を実施するパス（TOPSPIN path），（2）ICUでt-PA療法を遂行するパス（TOPSPEED：TOYOTA path for stroke with t-PA therapy after emergency evaluation and decision-緊急評価と決定後のt-PA療法のためのトヨタ脳卒中パス-），（3）ICU/SUで後療法を行うパス（TOPSTAR Jr.：TOYOTA path for stroke treatment, activity and rehabilitation judged by reexaminations-再評価で決定された治療，活動度，リハビリテーションのためのトヨタ脳卒中パス-）からなる，電子パス診療システム（3 TOP system）を構築した． 2007年4月1日の3 TOP system導入後，96例がTOPSPINで搬送され，11例でt-PAが投与された．TOPSPINと，これに連動した3 TOP systemは，t-PA療法の迅速・確実な診療に有効である．" @default.
