While the challenge of in utero gene transfer has evoked an extensive debate on the safety and practicality of this novel therapeutical approach, the impact of fetal gene therapy for the medical profession, however, has gained much less attention, even though the prospect of gene therapy in the womb promises to change the practice of prenatal medical treatment, fundamentally. A trained and experienced physician should assist the woman to make a fully free and informed choice between different therapeutic options. The treating physician bears the full responsibility for making such a recommendation but, based on the patient's autonomy, the woman reserves the right to finally decide which treatment strategy should be followed. The professional opinion of the physician must be based solely on the medical assessment of the patient's health interest, and requires an appropriate and complete consultation, so as to prevent hasty or ill-considered decisions Abstract Gene therapy, the treatment of disease by the transfer and sustained expression of an exogenous normal gene in somatic cells of a patient, is aimed to correct some types of severe genetic disorders. For most inherited genetic diseases, this therapeutic intervention should occur as soon as possible in early life before the onset of any fetal tissue damage. The rationale of fetal gene therapy is to transform a chronic, debilitating and often fatal disease into one that can be cured before birth, rather than merely to have it managed through the course of the patient's lifetime. In the past years, the challenge of in utero gene transfer has evoked an extensive debate not only on the safety and practicality of this novel therapeutical approach, but also a broad range of ethical and theological concerns. These controversies have focused mainly on ethical, legal, and social issues, such as questions concerning the ethical status of the embryo as a subject for clinical trials, and concerns were raised because of the violation of the genetic integrity of the fetus. Numerous publications relate to the balancing of risks and benefits for the mother and the fetus, the safety and efficacy of the approach, and the trans-generational risks to the germ-line. The impact of fetal gene therapy for the medical profession, however, has gained much less attention, even though the prospect of gene therapy in the womb promises to change the practice of prenatal medical treatment fundamentally. The present article focuses on the implications of in utero gene intervention for the patient-physician relationship and asks how the decision making in the context of fetal gene therapy will be balanced between the general practitioner, other health care professionals, and the mother. Furthermore, this paper asks how the acceptance of fetal gene transfer by different sections of the society will influence the attitude and expectations of both the mother and the treating physician towards abortion." @default.
