- W85395102 abstract "Aim. To evaluate the clinical and functional results of treatment of corneal dystrophy with active neovascularization in patients with congenital aniridia. Methods. 6 patients (12 eyes) were under observation with congenital aniridia, nystagmus, deep amblyopia, complicated corneal dystrophy with defects in the epithelium, disturbances of corneal transparency to the degree of opacification, corneal stroma edema, marked neovascularization from the periphery to the center of the cornea with complaints of low visual acuity and corneal syndrome, manifested by photophobia, lacrimation, and pain in the eye. 3 patients (6 eyes) received vasoproliferative factor inhibitors as subconjunctival injections, 5 patients (10 eyes), including 2 patients after a course of vasoproliferative factor inhibitors, had isolated erbium ytterbium laser coagulation of newly formed blood vessels of the cornea performed, which is a combined method of laser coagulation of newly formed blood vessels with phototherapeutic keratectomy and corneal collagen crosslinking. Results. Each of the used methods of treatment alone did not ensure 100% success. Only in one third of the cases of application of vasoproliferative factor inhibitors a permanent reduction of the severity of corneal syndrome was achieved, the activity of newly formed blood vessels was halved and the transparency of the cornea was increased. Laser coagulation led to a reduction by 50 80% of the newly formed blood vessels of the cornea, as well as the manifestations of the corneal syndrome; due to the regression of the corneal edema its thickness decreased by an average of 20-30 mkm. After phototherapeutic keratectomy with corneal collagen crosslinking during biomicroscopy the transparency of the cornea increased at the period of 2 to 4 weeks. Visual acuity with correction was in average 0.09 before intervention and 0.15-0.2 at 3 months thereafter. The cylindrical component of refraction decreased by an average of 3,25±1,12D. The thickness of the cornea, according to the OCT Visante, in the central zone decreased further by an average of 35±7 mkm. Conclusions. The combined method for treating corneal dystrophy with the growth of the newly formed blood vessels, including laser coagulation with phototherapeutic keratectomy and corneal collagen crosslinking, supplemented if necessary by subconjunctival administration of vasoproliferative factor inhibitors, is a safe method of treatment, which provides improved clinical and functional conditions of the eyes with aniridia associated with initial and moderate keratopathy with neovascularization. The method makes it possible to kill the pain, reduce the edema, increase the transparency of the cornea to improve visual acuity" @default.
