This phenomenon, termed “ischaemic preconditioning” [1] is the most powerful experimental method of delaying the onset of myocardial necrosis known so far, and its potent protective effects in the non-human heart are well established [2–4]. Of importance, the discovery has changed our perception of the consequences of myocardial ischaemia, and thereby of its major symptom, angina pectoris. In fact, angina, rather than always being harmful, may be regarded, in some specific clinical situations, as an indicator of increased myocardial resistance to lethal ischaemia [5–9]. In the clinical setting of acute coronary syndromes, and particularly of acute myocardial infarction (MI), the possibility that angina heralding MI may represent the clinical correlate of ischaemic preconditioning is extremely appealing because of its inherent cardioprotective action.
