- W86280350 abstract "Background : Epidermoid carcinoma is non melanoma skin cancer that has recurrency prevalence of 2 % to 8,9 % post wide excision. Factors which are known to influence the recurrency of epidermoid carcinoma are location that exposed sun, clinical staging, differentiation grading and excision margin. This study was aimed to asses factors associated with epidermoid skin carcinoma recurrence.Methods : This study was a historical cohort study. The samples were patients underwent surgery in oncology department of Kariadi hospital from January 1995 until December 2002. Data were analyzed using bivariate and logistic regression analysis. Survival analysis was conducted to measure the recurrency probability over a period of time.Result : A total of 89 samples were obtained. The recurrency prevalence was 21 %. Bivariate analysis found that factors associated with recurrency were differentiation ( RR = 13,8 ; 95 % CI = 3,97 — 47,61 ) , clinical staging ( RR = 11,9 ; 95 % CI = 3,47 —40,52 ), excision margin ( RR = 19,6 ; 95 % CI = 5,03 — 76,11 ), sun exposed time ( RR= 13,8 ; 95 % CI = 3,97 — 47,61 ) and sun exposed duration ( RR =31,2 ; 95 % CI = 6,47 — 150,18 ). Location was not associated with recurrence (RR = 3 ; 95 % CI = 0,36 — 25,04 ). Regression logistic analysis found that factors associated with recurrency were clinical staging ( RR = 13,3 ; 95 % CI = 1,2 — 146,2 ), sun exposed time ( RR = 11,9 ; 95 % CI = 1,5 — 96,7 ), sun exposed duration ( RR = 14,7 ; 95 % CI = 1,7 — 129,8 ) and excision margin ( RR =47,3 ; 95 % CI = 4,0 — 560,3 ). Differentiation was eliminated by others variable because there were significant association between differentiation and clinical staging, excition margin , time and duration of sun exposure.The recurrence probability after 4 years is 0 %.Conclusion : The factors associated with recurrency of skin epidermoid carcinoma are clinical staging , excision margin, sun exposed time and duration. The recurrence probability after 4 years is 0 %.Key word : skin epidermoid carcinoma, recurrence.Latar belakang : Karsinoma epidermoid kulit merupakan keganasan kulit non melanoma yang mempunyai prevalensi kekambuhan masih tinggi yaitu antara 2 % dan 8,9 % dengan batas eksisi pada jarak 2 cm dari tepi tumor. Beberapa faktor yang dianggap berhubungan dengan kekambuhan karsinoma epidermoid kulit yaitu lokasi tumor pada daerah yang sering mengalami paparan sinar matahari, stadium klinis , derajat diferensiasi dan batas eksisi. Penelitian ini ditujukan untuk mengetahui faktor — faktor yang berhubungan dengan kekambuhan karsinoma epidermoid kulit.Metoda :Penelitian ini menggunakan desain historical cohort . Sampel yang diambil adalah pasien karsinoma epidermoid kulit yang dioperasi di Bagian Bedah RSDK dari Jarman 1995 sampai Desember 2002. Data dianalisis dengan metode deskriptif dan analitik. Uji chi square digunakan pada analisis bivariat terhadap masing-masing variabel bebas untuk mencari besar Resiko Relatif ( RR) dan Confidence Interval ( CI ), sedangkan regresi logistik digunakan pada analisis multivarian terhadap semua variabel bebas yang hubungannya signifikan. Analisis kesintasan digunakan untuk mengukur proporsi kekambuhan pada jangka waktu tertentu.Hasil : Dari hasil peneliltian didapatkan 89 sampel yang memenuhi kriteria inidusi dan 21 % kambuh. Dan analisis bivariat didapatkan bahwa faktor yang berhubungan dengan kekambuhan karsinoma epidermoid kulit yaitu derajat diferensiasi ( RR = 13,8 ; 95 % CI = 3,97 — 47,61 ) , stadium klinis ( RR = 11,9 ; 95 % CI = 3,47 — 40,52 ), batas eksisi ( RR = 19,6 ; 95 % CI = 5,03 — 76,11) , waktu terpapar sinar matahari ( RR = 13,8; 95 % CI = 3,97 — 47,61 ) dan lama terpapar sinar matahari ( RR = 31,2 ; 95 % CI = 6,47 — 150,18 ), sedangkan lokasi anatomi tak berhubungan dengan kekambuhan KSS kulit ( RR = 3 ; 95 % CI = 0,36 — 25,04 ). Pada analisis regresi logistik didapatkan faktor-faktor yang berhubungan dengan kekambuhan karsinoma epidermoid kulit yaitu stadium klinis (RR = 13,2; 95 % CI = 1,2 — 146,2 ), waktu terpapar sinar matahari (RR = 11,9 ; 95 % CI = 1,5 — 96,7 ), lama terpapar sinar matahari ( RR = 14,7 ; 95 % CI = 1,7 — 129,8 ) dan batas eksisi ( RR = 47,3 ; 95 % CI = 4,0 — 560,3 ). Derajat diferensiasi tidak muncul dalam analisis regresi karena ada hubungan yang signifikan antara diferensiasi dengan stadium Minis , batas eksisi, lama dan waktu terpapar sinar matahari., sehingga variabel tersebut saling mengeliminir. Probabilitas kambuh setelah 4 tahun yaitu 0 %.Simpulan : Faktor-faktor yang berhubungan dengan kekambuhan karsinoma epidermoid kulit yaitu stadium klinis , batas eksisi, lama dan waktu terpapar sinar matahari, Kemungkinan kambuh setelah 4 tahun yaitu 0 %.Kata kunci : Karsinoma epidermoid kulit, kambuh." @default.
