- W863015906 abstract "2520 Objectives Myocardial perfusion imaging (MPI) is a well established technique in the diagnosis of ischemia and integral part in the management of coronary artery disease (CAD). Uniform reduction in blood flow due to diffuse 3-vessel disease in CAD may not be detected with MPI. Myocardial blood flow (MBF) measurements are sensitive to uniform changes in flow rate (expressed in ml/g/min) to myocardial tissue and can detect CAD. The aim of this study was to determine whether quantitative MBF adds benefit to standard Rb82 MPI and how such measurements correspond to anatomical coronary changes. Methods 128 patients with moderate likelihood of CAD underwent adenosine-stress/rest Rb82 PET/CT MPI, calcium score and coronary CT angiography. Rb82 MPI was visually assessed to determine the degree of myocardial ischemia. Myocardial flow reserve (MFR: the ratio of stress MBF/rest MBF) values for the major coronary arteries and a global value were derived by quantitative MBF analysis. Calcium score and CTA stenosis values were also quantitatively analyzed. Results 96 patients had normal and 32 abnormal Rb82 MPI scans. Of the 96 pts, 61 patients showed abnormal global MFR values (MFR Conclusions MFR quantification provides an added benefit in diagnosing CAD by highlighting compromised flow when visual assessment of Rb82 MPI images shows normal myocardial perfusion and no anatomical coronary changes can be found. For patients with abnormal Rb82 MPI scans MFR quantification can add confidence to the visual read." @default.
