- W866367621 abstract "Objective. to make consider Neurobrucellosis as the possible cause of neurological disorders especially in endemic areas where a patient may have a possible contamination or may have a history of Brucella infection in anamnesis.Background.In this case we present a 26 year-old patient who developed leukoencephalopathy with acute affection of 6th cranial nerve and with optic neuropathy . The symptoms starting from daily headaches and weakness progressed within 4 months leading to severe diplopia and blurred vision. In his past history he mentioned contamination with brucellosis about a year ago with full recovery according to clinical image and microbiological results.Methods. Diagnosis of Neurobrucellosis was based on clinical symptoms, cerebrospinal fluid examination with lymphocytic pleocytosis , positive Rose-Bengal agglutination test and Wright’s test with 1/200.T2-weighted cranial MRI revealed hyperintense diffuse white matter lesions especially in periventricular area and hippocampus atrophy.(images available).We started a 30 day treatment with the daily regim of Doxycycline 400 mg, Rifampicin 600 mg, and trimethoprim/ sulfamethoxazole 960 mg . A course of dexamethasone was added for 10 days after which one injection per week was remained for a month.Results.Within 3 weeks he achieved clinical recovery up to 80[percnt],and we plan a control MRI at the end of his treatment course.Conclusion.Neurobrucellosis is a severe form,a complication of systemic infection occurring in about 5[percnt] of all cases and leading to irreversible lesions if not treated in time.It is very important to follow up the patient’s symptoms especially in chronic forms of Brucellosis even in case of negative blood tests and in any suspect to pass a full neurological examination including cerebrospinal puncture.
