- W87367488 abstract "Resumen. La infeccion articular periprotesica (IAP) es una complicacion importante que requiere de investigacion y un plan preoperatorio ante ulterior cirugia. No hay situaciones en que exista una clara e inequivoca demostracion que oriente el diagnostico, pues hoy el cirujano ortopedico se enfrenta a unos casos cada vez mas complejos, predominando pacientes inmunocomprometidos, con enfermedades asociadas y bacterias resistentes a antibioticos. La aplicacion de profilaxis antibiotica se ha demostrado beneficiosa. Los germenes mas frecuentes productores de infeccion profunda son el estafilococus aureus y estafilococos coagulasa-negativos, como el estafilococus epidermidis. Deberan utilizarse, por tanto, cefazolina o cefuroxima en artroplastia de cadera o rodilla. El diagnostico puntual permitira llevar a cabo un tratamiento precoz. En ausencia de una prueba perfecta, la valoracion clinica, serologica (V/S, PCR), de imagen y un analisis bacteriologico, permitiran tener el diagnostico de infeccion, con un alto nivel de confianza. Sin embargo, hay que esforzarse en conseguir diagnostico microbiologico. La puncion articular aspiradora debera, por tanto, hacerse y repetirse si resulta negativa, con el fin de obtener mayor conocimiento bacteriologico. Infection of total joint arthroplasty. Infection of a total joint arthroplasty is a major complication that requires careful investigation and preoperative planning before further surgery, there are not areas where is clear unambiguous evidence to guide surgeons. We are currently faced with an increasingly complex case mixed, with a higher prevalence of immunocompromised patients, comorbidities and antibiotic-resistant bacteria. The use of antibiotic prophylaxis in articular replacement surgery has been shown to be beneficial. The most common organisms that cause deep wound infection are Staphylococcus aureus and coagulase-negative staphylococci such as Staphylococcus epidermidis. Therefore either cefazolin or cefuroxime should be used in conjunction with hip or knee arthroplasty. A prompt diagnosis permits early treatment. In the absence of a perfect test, a combined approach using the quadruple assessment of clinical evaluation, serological investigation (V/S, PCR), diagnosis imaging and microbiological analysis should enable the diagnosis of infection to be made with a high degree of confidence. Every effort should be made to achieve a definitive microbiological diagnosis. Therefore a preoperative aspiration should be performed and repeated if negative in the face of a high index of suspicion." @default.
