Review the anatomy, pathophysiology, and imaging findings including Octreotide scan, CT, and MRI of various gastroenteropancreatic neuroendocrine tumors. 2. Review pearls and pitfalls of Octreotide imaging. Endocrine tumors are rare neoplasms with malignant potential that have increased in incidence based on USA and European epidemiology data. Due to indolent growth and the fact that an estimated 33% to 40% of these tumors may be nonfunctional tumors, they are frequently diagnosed when disease has metastasized. Early detection and intervention is critical for improved prognosis and survival. Octreotide scan also referred to as somatostatin receptor scintigraphy (SRS) is the principal imaging modality in detecting gastroenteropancreatic and liver neuroendocrine tumors. Octreotide scan is based on tumor’s expression of somatostatin receptor (SSTR) type 2. There are five types of somatostatin receptors where most tumors express SSTR2 while small percentages express SSTR 1 and/or SSTR5. Not all tumors carry all five receptors and therefore may not be detected on conventional scintigraphic imaging. Additional radiology techniques may be required for identification, precise localization of increased scintigraphic uptake and/or to determine whether these Octreotide foci represent pathology or a benign entity/artifact. In this educational exhibit, we will review Octreotide imaging highlighting study technique and clinical indications, review approach to Octreotide study interpretation and discuss imaging findings of various neuroendocrine tumors and their respective differential diagnoses. Clinical and radiologic pearls along with common pitfalls and key points which can help in avoiding these pitfalls will also be discussed." @default.
