- W91069435 abstract "This chapter focuses on pharmacological issues relevant to the clinical application of bisphosphonates and to their use in the management of osteoporosis. Geminal bisphosphonates are synthetic compounds; they are analogs of natural pyrophosphosphate in which the oxygen has been replaced by a carbon. During early experiments bisphosphonates were shown to inhibit bone resorption, an observation that initiated their development as treatments for skeletal disorders. The effect of bisphosphonates on bone resorption is determined primarily by the structure of the R 2 side chain of the molecule, which is also referred to as the “bioactive moeity.” The mechanism of antiresorptive action of bisphosphonates has been a subject of intensive research for many years, but their molecular mechanism of action remained unknown until recently. Bisphosphonates share a number of pharmacological properties: low intestinal absorption, rapid elimination from the circulation, selective uptake by the skeleton, lack of circulating metabolites, renal excretion, and long skeletal retention. All bisphosphonates, independently of mode or route of administration, have been shown to significantly increase bone mineral density, predominantly of trabecular bone." @default.
