- W91385873 abstract "Rationale: Equity is a complex concept and various definitions are proposed from a number of different philosophical perspectives (e.g. Utilitarianism, Libertarianism etc.). Despite this complexity, in pragmatic terms there is widespread agreement that health care should be distributed according to need and financed according to ability to pay. The focus of this paper is on equity in Irish health care financing, defined in terms of ability to pay. Previous analysis using data from the late 1980s indicated a marginally progressive health financing system in Ireland. Objectives: - To update the results on equity in Irish health care financing by applying the international standard methodology to the most recently available public data. - To assess the suitability of this methodology for measuring equity in health care financing in the context where different entitlements are attached to different financing streams, using the Irish case as an example. Methodology: International standards for measuring vertical equity in health care financing are applied to 1999 Household Budget Survey data, replicating earlier analysis of 1980s data by Nolan (1993) and by Wagstaff et al. (1999). Vertical equity can be assessed in terms of the progressiveness of financing. Progressive financing occurs where payments rise as a proportion of income as income rises. One way to assess the progressivity of a specific source of health expenditure is to compare the share of the total financial burden borne by each income group with their share of total income. A progressive source of finance is one where the share of total health care payments borne by lower income groups is less than their share of total income. To make comparisons across sources of finance, summary measures of progressivity can be calculated using concentration curves. This analysis makes use of one such summary measure, the Kakwani index. Results: Results indicate that the sources of public health financing vary in progressivity. Income taxes are progressive while indirect taxes are regressive. Social insurance contributions (i.e. PRSI and 2% health levy) are progressive. For private health insurance, the concentration curves cross each other, leading to difficulties in interpreting the results. Out-of-pocket health expenditures are regressive although the discussion also indicates problems of interpretation. Overall, the results indicate a marginally progressive system, similar to the findings from the 1980s. Discussion: The paper discusses how the methodology provides an inadequate explanation of the progressivity/regressivity in private health insurance and in household out-of-pocket expenditure. The approach assumes that benefits from these financing sources contribute to the general public health system. Rather, the benefits from private sources of finance are largely restricted to those who pay. However, there may be knock on effects for the rest of the population depending on the structure of the health care system. In the Irish context, there are important interactions between the public and private health care systems that need to be taken into account when assessing the progressivity of private health care financing. Measuring equity in health care financing where different entitlements are attached to different financing streams is more complex than can be captured using summary concentration indices." @default.
