- W929147614 abstract "Background and Design.- In surgical neonatal intensive care units nosocomial infections and septicemia are still the leading cause of neonatal mortality. In our study 489 cases extending a period of 5 years was investigated retrospectively. Results.- 35.5% had clinical septicemia and in this group 76% had hemoculture positive septicemia. The mortality rate was 53%. In the first years, Enterobacteriaceae , Coagulase negative Staphylococci , Stenotrophomonas maltophilia sp. and Candida sp. Are mostly encountered but in recent years MRSA, Pseudomonas sp, Enterobacter sp. are most common microorganisms. Although antibiotic resistance in some resistant microorganism are seen more frequently, in our series we had a remarkable decrease in septicemia and related mortality. Conclusion.- As the intense use of modern technology in neonatal surgical intensive care units improved the fight against neonatal septicemia, neonatal mortality is still due to nosocomial infections and septicemia. Degisik serilerde cerrahi yeni doganlarda bakteremi orani %2-30 arasinda bildirilmektedir.1-4 Enfeksiyon kaynagi olarak cilt, yara enfeksiyonu, solunum sistemi bildirilmektedir.1 Etyolojik ajan olarak sorumlu tutulan mikroorganizmalar ise Staphylococcus aureus ve aerobik Gram-negatif basillerdir.1 Yeni doganlarin humoral ve selluler immun sistemleri iyi gelismemistir.2,5 Cilde ait savunma mekanizmalar inin da yetersiz olmasi nedeni ile ozellikle prematur bebekler enfeksiyona yatkindir.2,6-8 Buna altta yatan cerrahi hastalik nedeni ile anestezi ve cerrahinin enfeksiyonu arttirici riskleri de eklendiginde, cerrahi yenidoganlarda sepsis riski daha da artmaktadir. Ancak ameliyat oncesi ve sonrasi sepsis insidansi, etyolojik faktorler, sepsis, etken olabilecek mikroorganizmalar, tedavi ve sonuclari konusunda literatur bilgileri sinirlidir. Bu calismanin amaci cerrahi yenidogan yogun bakim unitesinde cesitli endikasyonlar ile yatirilarak tetkik ve tedavileri yapilan olgularda enfeksiyon etkenlerini, sepsis gelisim oranini ve risk faktorlerini saptamak ve mortalite iliskilerini geriye donuk olarak degerlendirmektir." @default.
