• W930031208 abstract "Issues about ethnicity and spirituality do not apply to the study of genetic deafness alone. Much of what we will be discussing in this chapter applies to other areas of health care, such as palliative care and end of life issues as well as those surrounding childbirth and pregnancy itself. Genetic counselling raises particular concerns as discussed with reference to deafness by Middleton and Belk in this volume and by authors such Ahmed, Green and Hewison (2005) and Ahmad and Atkin (2003) in the context of thalassaemia. There is a core of central beliefs about health care provision which rests on the appropriate and sensitive delivery of health care within a system based on equity and quality. Inequalities in health extend to other areas besides ethnicity and the picture is a complex one if age, education, gender, income, disability and sexuality are taken into consideration. Ethnicity does not rest on nationalism alone or colour of skin. The very notions of ‘race’ and ethnicity are challenged and endlessly redefined (as discussed later on). Spirituality too extends beyond the boundaries of organised religion to include people’s faith and beliefs when challenged by difficulties such as receiving the news of a genetic history affecting the potential birth of a child. The sense that we make of the problems we encounter is influenced by the beliefs we have about the meaning (or otherwise) of life. This chapter is based on research on the relevance of spirituality to management of chronic conditions within the Pakistani community Mir (2005), and on health care delivery to deaf children and adults from minority ethnic communities (Chamba, Ahmad & Jones, 1998; Ahmad, Darr & Jones, 1998) as well as on the practice of genetic counselling in a large Northern town in the UK (Khan). It highlights the importance of understanding ethnicity and faith (both aspects of culture) in the policy, practice and study of genetic counselling in deafness. To do this we look at genetic counselling in deafness and ways of changing practice to encompass ethnic, religious and cultural difference and at a study of Pakistani Muslims in the UK as a specific example.
