- W93695589 abstract "パージェタ点滴静注420 mg/14 mLは，米国のGenentech, Inc.（以下，Genentech社）により創製された遺伝子組換えヒト化モノクローナル抗体，ペルツズマブ（遺伝子組換え）を有効成分とする点滴静注用製剤である．ペルツズマブはヒト上皮増殖因子受容体2型タンパク質（HER2：human epidermal growth factor receptor type 2）の細胞外領域で二量体形成に関与するドメインIIに結合することにより，HER2-HER3等の二量体形成を阻害し，ヘテロダイマーからのリガンド依存性シグナルを遮断する．一方，トラスツズマブは，HER2タンパク質の細胞外領域で比較的細胞膜に近いドメインIVに結合し，リガンド非依存性シグナルを遮断する．ペルツズマブとトラスツズマブはHER2への結合部位，作用メカニズムが異なることから，両剤を併用することでより広範なHER2シグナル遮断を通じて相乗的な抗腫瘍効果が得られると考えられている．ペルツズマブとトラスツズマブの併用は，HER2高発現ヒト乳がん株KPL-4移植モデルを用いた非臨床試験において高い抗腫瘍効果を示し，トラスツズマブによる治療中に増悪したHER2陽性転移・再発乳がん患者を対象とした第II相臨床試験（BO17929試験）においてその有効性が示された．これらの成績を受けて，HER2陽性転移・再発乳がんを対象に，初回治療としてトラスツズマブ+ドセタキセルにペルツズマブあるいはプラセボを併用する国際共同第III相臨床試験（CLEOPATRA試験）が実施された．その結果，主要評価項目である独立判定機関評価による無増悪生存期間および副次的評価項目である全生存期間の有意な延長が認められた．また，ペルツズマブ併用の有無により有害事象の発現率，有害事象による死亡および投与中止の頻度に大きな違いが認められず，ペルツズマブの忍容性が確認された．これらの成績に基づき，2012年6月に米国，2013年3月に欧州にてHER2陽性転移・再発乳がんの一次治療として承認された．また，本邦では2013年6月に「HER2陽性の手術不能又は再発乳癌」の効能・効果で承認された．" @default.
