- W957079533 abstract "Alterations in the endocrine environment which are prominent features of the uremic syndrome are associated with a variety of hormonal and metabolic disturbances (Feldman and Singer 1974; Wilson 1971; Emmanouel etal. 1980). In some instances, these derangements persist despite effective dialysis and adequate biochemical control, improving only with successful kidney transplantation, implicating a nonfunctioning kidney rather than the uremic environment in the pathogenesis of the metabolic disturbances. However, in other cases striking improvements occur following dialysis, suggesting that the disturbed metabolic environment of uremia and not loss of a particular renal action is responsible. In either case, the kidney clearly plays a central role in determining the prevailing endocrine milieu. There are a number of possible mechanisms by which the kidney may be responsible for disturbances of the endocrine and metabolic status. These include: (a) producing a hormone (erythropoeitin); (b) activating a prohormone (1,25-dihydroxyvitamin D); (c) stimulating release of a hormone (renin → angiotensin →aldosterone); (d) subserving an important catabolic role in the metabolic disposal of hormones (glucagon, insulin, parathyroid hormone, calcitonin, prolactin, and a number of anterior pituitary and gastrointestinal hormones); and (e) uremia– associated interference with the extrarenal degradation of hormones.
