- W95932219 abstract "Introduction Radiation therapy after nodal surgery for melanoma remains controversial. Most reported studies are retrospective reviews of single institution experience. This report describes the outcomes in patients with high risk melanoma who have received postoperative radiation therapy using standardised guidelines after nodal surgery and is unique in being both prospective and multicentred. Methods Patients were required to have had nodal surgery in the head and neck, axilla or ilioinguinal node basins with no evidence of distant metastases. Eligible patients included those with multiple node involvement, extranodal spread, a nodal mass larger than 4 cm, tumour spill at surgery or previous nodal surgery to the area. Radiation was performed using standardised techniques. The primary endpoint was local in-field relapse. Secondary endpoints were adjacent and distant relapse, toxicity, progression-free and overall survival. Results 234 patients were enrolled over an 11 year period from 7 institutions in Australia and New Zealand. The median age was 57 years and 73% were males. Unknown primaries occurred in 17% and 75% had extranodal spread. More than 2 nodes were involved in 51%. One operation had been performed in 87%. The local in-field relapse rate as the first site of recurrence was 6.8%. Adjacent and distant failures occurred in 13.6% and 62.8% respectively. Unknown primary, node site, number of involved nodes, extranodal spread or multiple operations did not significantly alter patterns of relapse. The overall 2 year survival was 48%. Only the presence of more than 2 nodes involved had a significant effect on survival. Conclusion We conclude that postoperative radiation therapy to the nodal basin in feasible using standardised techniques in a multicentre setting. Comparison with surgery alone series or other studies of postoperative radiation therapy is difficult. A randomised trial is currently being performed to ascertain the true benefit of adjuvant radiation therapy." @default.
