- W96325111 abstract "Symptoms attributable to reflux of gastric contents from the stomach into the esophagus are recognized by almost all individuals as something that they “think” they have experienced at one time or another. These symptoms are the subject of jokes and are perceived by most as an annoyance, not a disease or even a potentially serious problem. When closely examined, however, our understanding of gastroesophageal reflux, from its etiology to the degree of injury that it may produce, has increased to the point where we now recognize that a variety of serious medical problems may occur secondary to this phenomenon. Even with this understanding our knowledge is at best limited and undergoing extremely rapid change as this whole concept is subjected to very careful examination and clinical investigation. This chapter will review our present understanding of why reflux of material from the stomach into the esophagus occurs, discuss what factors are responsible for the variability in response to this noxious material, consider whether the esophagus is the only site of injury, review the material supporting the claim that the consequences of gastroesophageal reflux are more widespread and potentially clinically significant than has heretofore been recognized, and finally, on this basis, consider how we can effectively treat individuals with this problem, either medically or surgically.
