- W97900719 abstract "In Sweden the training of nurse midwives in family planning began 10 years ago, mostly in response to a lack of physicians prepared to perform the task. A postgraduate training program for midwives in contraceptive services was initiated in 1972. After the course or equivalent training in basic education, a midwife has to do inservicce training, following which she is allowed to run independently a family planning unit providing information and contraceptives. In the event of complications she has to consult or refer to a doctor responsible for maternal health care. In Sweden oral contraceptives (OCs) are not available without prescription. Initially, the trained midwives were not allowed to fill such prescriptions. In 1978, following discussions with physicians' professional organizations and with support from obstetricians and gynecologists who were familiar with and appreciated the way midwives worked, the law was changed and midwives with appropriate training were given the legal right to prescribe medical drugs for contraceptive use, including OCs and injectables. At first there was some anxiety among trained midwives about their capacity to take on the doctor's role in this area. Additionally, they were concerned about whether doctors and patients would accept them as advisers. Soon it became evident that nurse midwives did a good job and were liked both by the doctors they worked with and by their contraceptive clients. Midwives are recognized as good advisers due to the psychological sensitivity of their approach and their familiarity with clients. They are easy to reach for an appointment or telephone consultation. The fact that most of them are women also contributes to their popularity with other women. As a result the nurse midwives frequently obtain better results in respect of acceptance and continuation rates than physicians working in this field. There are about 3000 nurse midwives in Sweden almost 1/3 of whom are working in antenatal care in the maternal health system. By now, 1000 midwives have contraceptive inservice training and the right to prescribe contraceptives. Most of them also provide family planning services within the maternal health system.
