- W985246876 abstract "Heart failure is one of the commonest causes of death in the world. Despite advances in medical treatment, the management of the patient with end-stage heart failure is based on the replacement of the heart by transplantation of a new organ, but this has been restricted by the shortage of donors. The replacement of the heart by a total artificial heart is a procedure that still remains experimental. An alternative strategy to restore cardiac function can be the regeneration of the diseased myocardium by transplantation of cell lines that can differentiate into muscle. In this regard, it has been demonstrated in animal models of heart failure that a diverse range of cell types such as skeletal myoblasts,1 bone marrow,2 fetal and neonatal cardiomyocytes,3,4 and even autologous heart cells5 can improve cardiac function. There is now considerable experimental evidence that transplantation of bone marrow cells into the heart induces angiogenesis and improves contractility6-9 but little is known of its effects in man. The use of the patient’s own cells for the regeneration of cardiac muscle has the advantages that it is ethically acceptable and immunosupression is unnecessary. The use of unmanipulated bone marrow cells as opposed to myoblasts and other cell types has the additional benefit of ready availability, without the need for in vitro cell culture preparation. In this chapter, the results of a phase I, non-randomized study performed by my group in which unmanipulated autologous bone marrow cells were injected into the scarred myocardium of patients undergoing coronary bypass surgery are presented.
