- W99169823 abstract "252 Objectives: While 82Rb PET has better image quality, depth-independent resolution and fewer attenuation artifacts than [99mTc]sestamibi SPECT (Bateman, J Nucl Cardiol 2006) and does not require a cyclotron as does [13N]ammonia PET, the extremely short half-life of 82Rb leads to imaging challenges. Thus, 82Rb PET calls for determining the amount of activity to inject that balances the advantage of a high true coincidence rate against the detrimental effects of high randoms and dead-time. Herein we describe a quantitative method to assess this tradeoff and suggest an imaging protocol for a LYSO PET/CT scanner. Methods: The optimal injected dose was determined according to the noise equivalent counts (NEC) which is calculated in terms of trues, randoms and scattered events using data between 2 and 6 minutes after injection. The dependence of these three event rates on injected activity in individual subjects is predicted using an improved version of the [18]FDG count-rate model proposed by Watson (J Nucl Med 2005) while accounting for performance characteristics of Gemini TF PET/CT (Philips). Results: In data from 56 scans in 29 subjects (wt: 81 kg , ht: 1.64 m, BMI 29 kg/m2) retrospectively analyzed, the optimal injected dose was predicted to be 1.1 to 1.5 GBq in 68%, > 1.5 GBq in 23% and Conclusions: The predicted 1.1 to 1.5 GBq optimal dose range is consistent with the range recommended by the American Society of Nuclear Cardiology (Machac, J Nucl Cardiol 2006) for 3D PET/CT using lutetium-based detectors although we are using a longer 2 vs. 1.5 minute delay after injection. The method presented here can be used to optimize patient dose based on NEC of clinical scans while accounting for specific scanner and imaging protocol details.
