Often the gastrointestinal obstruction represents a turning point into the transition of the last phase of life. The nurse is responsible for providing sensitive anticipatory guidance on what to expect. The advanced practice nurse or case manager is in the ideal position to coordinate care with other services such as home care, hospice, and long-term care facilities. Referrals and consultation with other professionals such as the wound, ostomy, continence nurse (WOCN)/ enterostomal therapy (ET) nurse, social worker, pastoral care worker, psychologist, and palliative care nurse practitioner are helpful in meeting the multidimensional needs of the patient and family. More research is needed to study the palliation and treatment of gastrointestinal malignant obstructive disorders. Clinical trials are needed to determine which pharmacologic agents work the best for this patient population. Studies documenting the amount of nursing time and skills required for symptom management of those suffering with obstructive disorders are important for planning staff quota and reimbursement justification. Nursing research needs to evaluate the effectiveness of current interventions so that evidence-based palliative nursing care can be provided." @default.
